I represent and warrant that (a) I am the parent or legal guardian of the minor named above, (b) no court has issued any order, judgment, or decree granting custody of the minor to anyone else or otherwise affecting my rights as parent or legal guardian, (c) the minor has not been emancipated, (d) I have the legal right, power, and authority to consent to this Agreement on behalf of the minor and myself, and (e) I am at least 18 years of age. I hereby grant the Sisters of the Holy Cross permission and a perpetual, irrevocable, worldwide and non-exclusive license to reproduce, distribute, and display the student’s essay/art submission (the “Submission”) in the Essay Contest (“the Contest”), without charge, in any media format and all manners whether now existing or hereinafter developed, including, but not limited to: Sisters of the Holy Cross publications, presentations, website, social media, and/or other communication channels. I agree that the essay/art submission may be used for a variety of purposes without any notification and compensation. I understand the Sisters of the Holy Cross might use the student’s full name, the school that he/she attends, and the student’s grade, without first obtaining my expressed permission. I hereby waive and release all claims I and the student may have against the Sisters of the Holy Cross, for myself and the student and our personal representatives, heirs, successors, and assigns related to the Submission, as well as the other information set forth therein, in consideration for being involved in the Contest and the opportunity to potentially receive a prize, hereby hold harmless, release, indemnify and discharge the Sisters of the Holy Cross, including for any claims for defamation, disparagement and/or invasion of privacy.
